Medical Disclaimer & Practitioner Declaration

This document forms part of the Terms of Service and is legally binding.

This Medical Disclaimer applies to all users of TeqCliniq, a clinical software product owned and operated by Clinicwave Innovations Private Limited.

IMPORTANT — PLEASE READ CAREFULLY

TeqCliniq is a clinic management software tool. It does not provide medical advice, diagnosis, or treatment recommendations.

The Platform does not:

Verify the accuracy or appropriateness of any prescription
Check for drug interactions or contraindications
Validate diagnoses against clinical guidelines
Verify the medical qualifications of registered users
Provide any form of clinical decision support

All clinical decisions are the sole and exclusive responsibility of the registered medical practitioner.

Solemn Declaration

I, the undersigned registered medical practitioner and citizen of India, hereby solemnly declare and affirm that:

1. I am a registered medical practitioner holding a valid registration under the National Medical Commission (NMC) Act, 2019 or the respective State Medical Council of India, or I am a student enrolled in a recognized medical institution.

2. My Medical Registration Number provided during registration is valid, active, and not suspended or revoked by any medical regulatory authority.

3. I shall use TeqCliniq solely for legitimate clinical practice in accordance with the Indian Medical Council (Professional Conduct, Etiquette and Ethics) Regulations, 2002.

4. I am solely responsible for all prescriptions, diagnoses, treatment plans, and bills generated through this Platform. TeqCliniq bears no responsibility for the accuracy or appropriateness of my clinical decisions.

5. I shall protect the privacy and confidentiality of all patient data entered in this system as per my duty under the Indian Medical Council Regulations and the Digital Personal Data Protection Act, 2023.

6. I shall not transfer my login credentials to any unqualified person, and I shall ensure that Staff members assigned "Doctor" roles under my account hold valid medical registrations.

7. I shall not use this Platform for generating fraudulent bills, false insurance claims, or any purpose that violates the Indian Penal Code (Bharatiya Nyaya Sanhita, 2023) or any applicable law.

8. I understand that violation of these terms may result in immediate termination of my account and may be reported to the relevant medical regulatory authority and law enforcement agencies.

9. I understand that TeqCliniq is a software tool and not a medical service provider, and I shall not hold TeqCliniq liable for any clinical outcomes resulting from my use of this Platform.

10. I affirm that all information provided by me during registration is true, accurate, and complete to the best of my knowledge and belief.

By creating an account on TeqCliniq, you confirm acceptance of this declaration in accordance with the Indian Evidence Act, 1872 (Bharatiya Sakshya Adhiniyam, 2023) and the Information Technology Act, 2000.
